Welcome to Janesville-Waldorf-Pemberton High School Activity Registration

Step 1: Athletes must have a current sports qualifying physical on file. Physicals are good for three years from the date of your last physical. (If you are unsure of your students physical expiration date please call the high school office, 507-231-7777). Physical forms can be downloaded here. Please take this form with you to your doctors appointment. Once you have your physical complete and signed by the doctor, you will need to scan it and upload it into the registration platform. If you currently have a physical on file from a previous year and it has not expired there is no need to complete this step. Typically all 7th and 10th graders need physicals as do new students. *It's important that you fill in correct dates for your athletes physicals.*

Step 2: Fill out and submit the Online Registraion Form (including all necessary forms). You will need to register for each season. The first time you register you will select, "New User". For any future registrations you will select "Returning User", and the process is much quicker as your information will be saved. 

Step 3: Pay your activity fee online or make arrangements with the Activities Office if payments cannot be made online. Students will not be cleared to participate in games until the fee is paid. You will receive a thank you message confirming that the form was completed and sent to the JWP Activities Department.

Step 4: Check your account periodically for updates and additonal paperwork that may need to be signed. You will get a notification from the high school office if new paperwork is available to sign.
